- Log in to the TRT.
- On the Home page, under Administer Transition Tools, click Printable Tools.
- Locate the evaluation you want to download.
- Click the download icon under Self-Report or Guided.
- A PDF preview will open in a new tab.
- Click the Download button to save the file to your computer.
Self-Report tools are completed independently by the student, while Guided tools are designed to be completed with support from a provider or instructor.
Evaluations are also available with face icons, which provide extra visual support for students.

- ZIP Files: Scroll down to download multiple tools at once using the ZIP file links.
- Other Languages: Scroll to the Alternative Languages section to download printable tools in Spanish, French, Chinese, and other languages.
