TRT Overview
The Transition Readiness Toolkit (TRT) is a suite of evaluations designed to measure students’ self-reported confidence and readiness across multiple transition-related domains. Each evaluation asks participants to respond to statements using a four-point scale, which is then converted into standardized 0–100 scores for consistency and comparability.
Response Scale
Response Option | Numeric Value | Meaning |

Each question is scored 0–3, and item-level scores are averaged and scaled to a 0–100 range for reporting.
For example:
- Mean raw score of 3.0 = 100
- Mean raw score of 1.5 = 50
- Mean raw score of 0 = 0
This linear transformation provides a standardized confidence metric across all TRT evaluations.
Transition Planning Tool
The TPT is one of the evaluations within the TRT. It consists of 45 items total, divided evenly across nine service categories (5 items per category). Each service category yields a score from 1 to 10, reflecting the student’s level of confidence within that category. These category-level scores can then be used individually or aggregated for summary reporting and analysis.
